We have disabled access and do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance or medical conditions.
Please bring in your medical card. If you are unable to provide this you must complete a form for yourself and any other family members wishing to register. (New patients over 16 must sign the form themselves.) You will need to provide some from of ID and proof of residency. If you have previously been registered with a doctor in the UK you will need to let us know your NHS Number - this can be obtained from your previous GP. Forms are available from reception.
If you have not been registered with a doctor in this country, you will be asked to bring in official documents and proof of address.
You will be offered a health check with one of our practice nurses or health care assistants. This check is very important as the practice will then have some information about you.
If you are on regular medication please bring this with you.
If you have children less than five years of age they do not need a health check but it is imperative that you bring in their Child Health Record Book and record of immunisation.
You will be registered with the practice rather than an individual doctor and therefore free to see any doctor.
